Duties of this position include following, in accordance with established policies and therapy standards:

  • Perform evaluations
  • Develop and implement effective patient treatment plans to restore, compensate or adapt for loss of patient function.
  • Plan and administer medical prescribed therapy treatments.
  • Evaluates effects of therapy treatments by observing, noting and evaluating resident’s progress.
  • Manages the appropriate therapy minutes per RUGS category for patients/residents.
  • Assures continuation of therapeutic plan following discharge by designing resident specific maintenance program

Candidates for the Physical Therapist and/or Physical Therapist Assistant – PT/PTA position should meet the following requirements:

  • Graduate from an accredited program for Physical Therapy.
  • Meet educational requirements established in the state of practice.
  • Current licensure in practicing state and/or nationally certified.
  • Possess problem-solving skills.
  • Must be able to relate professionally, positively, and cooperatively with patients, patient’s family members and the facility staff.
  • Must have own transportation to travel between sites as needed.
  • Must meet Clinical Competency requirements upon hire and as needed.
  • Must meet all local health regulations, pass post-offer drug test, and post-offer employment physical exam if required.


  • Utilize the nursing process to deliver quality, outcome-focused care
  • Participate in developing, implementing, evaluating, and revising the Plan of Care for each resident
  • Establish and maintain positive interactions with residents, families and other staff and serve as the primary point of contact for family members
  • Keep other healthcare professionals informed of residents’ conditions
  • Ensure proper documentation to meet all reporting requirements
  • Promptly respond to call lights and other resident needs
  • Demonstrate strong ethical and safety practices
  • Maintain knowledge in pharmacology, lab values, and infection control
  • Participate in peer support / evaluation, in services, education programs, and staff meetings, and Quality Improvement activities as assigned

Job Requirements for Registered Nurse – RN:

Candidates for the RN position should meet the following requirements:

  • Hold and maintain a current state license to practice as a Registered Nurse.
  • Recent experience or education specific to geriatrics.
  • Self-reliant and self-aware.
  • Ability to combine common sense with clinical skills to ensure optimal patient care.
  • Demonstrate understanding of relevant government regulations.
  • Ongoing pursuit of Continuing Education Credits in clinical subjects, management, personal growth and development.
  • Working knowledge of infection control principles and practices.
  • Ability to communicate effectively and positively in all interactions.
  • Capable of maintaining regular attendance.
  • Meeting all local health regulations and pass post-employment physical exam, drug screening, criminal background investigation and reference inquiry if required.
